I, (NAME ABOVE), do knowingly, voluntarily and willingly agree to assume ALL LIABILITY AND RISK associated with any activity that I (in the event I am an adult participant) or my child engages in, of whatever nature and in whatever scope and place, during his/her participation in, or while on any premises used for/by Loren Riddick, Loren Riddick DBA as Stars of Tomorrow Basketball Camp, his agents or assigns, or businesses in which he holds any interest, wherever situated, as well as The City of Alcoa and the Alcoa School System, and any Sponsors of the Basketball Camp. I further acknowledge that any participation or activities that I ( in the event I am an adult participant) or my child performs during this Basketball Camp carries with it the risk of injuries and as such, I understand that I (in the event I am an adult participant) or my child may be injured in the course of activities pondered in this Basketball Camp. I knowingly, voluntarily and willingly HOLD HARMLESS Loren Riddick, Loren Riddick DBA as Stars of Tomorrow Basketball Camp, his agents or assigns, or businesses in which he holds any interest, as well as The City of Alcoa and the Alcoa School System, and any Sponsors of the Basketball Camp. for any injury, damage or any other mishap or accident involving my child or as their parent or guardian, or I, myself (in the event I am an adult participant). I assume all risk for any such injuries or mishaps, or damages to which I (in the event I am an adult participant), my child, or as their parent or guardian might incur during the course of my participation in the Basketball Camp.
